Third-Party Release Form
A third-party release form is used so you can authorize someone else (typically family members) to have access to your education records/information.
Email the form to: registrar@byui.edu
Missionary Privacy Release Form
This form allows parents to have a Password Reset option while you (the student) is on a mission: Mission Privacy Release Form This form is used exclusively by BYU-Idaho's IT HelpDesk and is to be used only for extenuating circumstances. Students must contact the BYU-Idaho HelpDesk for additional information. Financial Aid Third-Party Release Form
Privacy Release forms for Financial Aid information are handled by the Financial Aid Office.
